Here is my form for Application/Order to Appear when the defendant resides outside of the suit county.  This contains the language that Sedgwick County now requires in this situation.

     1.  Judgment was entered on 04/01/97, in favor of said
plaintiff against said defendant(s) for the sum of $1000.00,
interest and costs.

     2.  That either execution or garnishment has been issued in
this matter and has been returned unsatisfied in whole or in
part, or the plaintiff is without sufficient knowledge of the
debtor's assets to cause an execution be issued.

     3.  The judgment debtor resides outside this county, but has
submitted to the jurisdiction and venue of this Court, and that
requiring said judgment debtor to appear before this Court for
such examination will not cause undue hardship to said judgment
debtor.
